According to the folks at Epicurious, go to Oahu and you'll find loads of restaurants dishing out bunless hamburgers smothered in gravy. Sounds a little like Salisbury steak to us, but perhaps the whole consuming-it-in-paradise thing makes a big enough difference to warrant a name change.
